What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.

Generics are less expensive because generic manufacturers don't have to invest large sums of money to develop a drug. When the brand-name patent expires, generic companies can manufacture a copy of the brand-name and sell it at a substantial discount.

Description

Zeposia (Ozanimod) is a sphingosine 1-phosphate receptor modulator, which means it acts on specific receptors in the immune system to regulate its functions. Zeposia uses include the management of relapsing forms of multiple sclerosis (MS), including clinically isolated syndrome, relapsing-remitting disease, and active secondary progressive disease in adults. Zeposia may also be used to treat moderately to severely active ulcerative colitis.

Directions

When taking Zeposia, patients should follow their healthcare provider's instructions. Typically, the patient starts with a lower dose and gradually increases to the full maintenance dose over a week's time, known as a titration period, to reduce the risk of heart-related side effects. Zeposia comes in a pill form and is taken once daily, with or without food. Always consult a healthcare provider for the appropriate dosage for treatment.

Ingredients

Zeposia's active ingredient is Ozanimod.

Contraindications

Avoid taking or using this medication if any of the following apply:

  • Patients who have a known hypersensitivity to Ozanimod or any of its components should not take Zeposia.
  • Those who have experienced a myocardial infarction, unstable angina, or stroke within the last six months.
  • Individuals taking monoamine oxidase inhibitors (MAOIs) such as isocarboxazid, linezolid, phenelzine, procarbazine, rasagiline, selegiline, selegiline transdermal, or tranylcypromine should not use Zeposia, as harmful interactions may occur.
  • Those with untreated sleep apnea should not use Zeposia.

Cautions

  • Consult a healthcare provider about the following before taking Zeposia:
    • Any existing allergies
    • Whether you are pregnant or breastfeeding
    • Other medical conditions you have
    • All current medications you are taking
  • Zeposia can increase your susceptibility to infections. Before beginning treatment, a complete blood count (CBC) is necessary to ensure your immune system is ready. It's crucial to monitor for any signs of infection both during the treatment and for three months after you've stopped taking Zeposia. Starting Zeposia isn't recommended if you currently have an active infection.
  • There's a rare but serious risk of developing a brain infection known as PML. If you notice symptoms like sudden weakness, changes in vision, or confusion, stop Zeposia immediately and get in touch with your healthcare provider.
  • When starting Zeposia, you might experience a temporary decrease in heart rate. An ECG (electrocardiogram) will be done before you start the medication to check for any heart issues. Those with existing heart conditions may need a consultation from a cardiologist, especially when Zeposia is used alongside other medications that affect the heart rate.
  • Zeposia could cause liver damage. Liver function tests are required before beginning treatment. If significant liver injury is detected, Zeposia will be discontinued.
  • Women who can become pregnant should use effective birth control during their Zeposia treatment and for three months after the final dose due to potential risks to the fetus.
  • Regular monitoring of your blood pressure is advised since Zeposia may cause it to rise.
  • Zeposia has the potential to affect lung function negatively. Pulmonary function tests, like spirometry, might be conducted if there are clinical indications.
  • Any change in vision warrants an immediate eye exam. Those with diabetes or a history of uveitis are at an increased risk of developing macular edema and should have a detailed eye assessment before starting Zeposia.

Side Effects

Zeposia may cause cold-like symptoms, dizziness when standing, back pain, headaches, liver enzyme increases, urinary discomfort, and high blood pressure. Serious Zeposia side effects include liver problems, which may require regular liver function tests. Symptoms to watch for include nausea, yellow skin or eyes, and abdominal pain.

Other serious risks include elevated blood pressure, trouble breathing, and vision problems such as macular edema. PRES, a rare brain condition, could occur, potentially leading to stroke if not managed. Consult a healthcare provider if any serious side effects develop, persist, or worsen.
