What is a Generic Drug?

Description

XARELTO (Rivaroxaban) is a medication used to treat or prevent the development of clots in certain patients. It can be prescribed for the following conditions:

  • Prevention of clot-related major cardiovascular events in people with Coronary Artery Disease (CAD) or Peripheral Artery Disease (PAD)
  • Prevention of the formation of clots in people with Non-valvular Atrial Fibrillation
  • Prevention of the recurrence of Deep Vein Thrombosis (DVT) or Pulmonary Embolism (PE)
  • Prophylaxis of clot formation in people hospitalized for an acute medical condition and are at a high risk of Venous Thromboembolism
  • Prophylaxis of Deep Vein Thrombosis (DVT) following hip or knee replacement surgery
  • Treatment of active Deep Vein Thrombosis (DVT)
  • Treatment of Pulmonary Embolism (PE)

XARELTO works by inhibiting factor Xa (Ten A), which is a critical protein in the clotting cascade. Inhibition of factor Xa decreases the risk of clot formation (Venous Thromboembolism or Deep Vein Thrombosis). Clots can break off and cause major ischemic events like a cardiovascular accident (stroke), myocardial infarction (heart attack), and pulmonary embolism. Prevention of clot formation has been shown to reduce the risk of these events.

You may take XARELTO for any length of time between three months to lifelong, depending on your clot risk and the determination by the physician.

Directions

The dosage of XARELTO depends on why you are taking it:

  • Non-valvular Atrial Fibrillation: Take 20 mg once daily with food.
  • DVT or PE treatment: Take 15 mg twice daily with food for 21 days, and then take 20 mg once daily with food.
  • DVT or PE recurrence prevention: Take 10 mg once daily with or without food following 6 months of standard anticoagulation treatment.
  • Prophylaxis for major cardiovascular events in CAD or PAD: Take 2.5 mg twice daily with or without food along with aspirin.

Doses of XARELTO greater than 10 mg should be taken with food to ensure full absorption of the drug.

If you have mild to moderate renal impairment, your physician may prescribe a reduced dose of XARELTO. It should not be taken by those with severe renal impairment.

Cautions

Speak with your physician before discontinuing XARELTO, as doing so may temporarily increase the risk of clot formation.

The risk of fatal and non-fatal bleeding events is increased while taking XARELTO, which should be weighed against the risk of clotting events. XARELTO should be used with caution with other drugs that impair hemostasis like NSAIDs, SSRIs, SNRIs, antiplatelets, antifibrinolytics, and other antithrombotics.

The dosage of XARELTO will need to be adjusted in people with renal impairment. The drug should be avoided in those with hepatic impairment.

XARELTO and other factor Xa inhibitors have not been established as safe and effective in patients at an increased clot risk due to heart valve replacements or in those with triple-positive antiphospholipid antibody syndrome.

Side Effects

Since XARELTO is intended to prolong the amount of time it takes blood to clot, the most common side effects will be related to bleeding, and may include:

  • A decrease in hemoglobin caused by bleeding
  • Bruising
  • Fatal bleeding
  • Gastrointestinal bleed
  • Hemorrhagic stroke
  • Intracranial hemorrhage (ICH)
  • Non-major bleeding

Other non-bleeding-related side effects include:

  • Abdominal pain
  • Anxiety
  • Back pain
  • Blister
  • Depression
  • Dizziness
  • Fatigue
  • Insomnia
  • Muscle spasm
  • Pruritis
  • Syncope
