What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.

Description

Trijardy XR contains Jardiance (empagliflozin), Tradjenta (linagliptin), and metformin hydrochloride (HCl). This three-drug combination medication is used with diet and exercise to improve A1C in adults with type 2 diabetes.

Directions

Trijardy XR (empagliflozin, linagliptin, and metformin hydrochloride extended-release) is supplied as a tablet in various strengths:

  • 5 mg empagliflozin/2.5 mg linagliptin/1000 mg metformin HCl extended-release
  • 10 mg empagliflozin/5 mg linagliptin/1000 mg metformin HCl extended-release
  • 12.5 mg empagliflozin/2.5 mg linagliptin/1000 mg metformin HCl

  • 25 mg empagliflozin/5 mg linagliptin/1000 mg metformin HCl extended-release

Your current treatment regimen and kidney function will help determine your appropriate starting dose.

Trijardy XR is taken by mouth once daily with a morning meal. The tablet should be swallowed whole. No more than 25 mg empagliflozin, 5 mg linagliptin, and 2000 mg metformin HCl should be taken per day.

Ingredients

Empagliflozin, linagliptin, and metformin hydrochloride are the active ingredients contained in Trijardy XR.

Contraindications

Avoid taking Trijardy XR if you have severe kidney problems, end-stage kidney disease, or if you are on dialysis.

Avoid taking Trijardy XR if you have metabolic acidosis such as diabetic ketoacidosis.

Avoid taking Trijardy XR if you are allergic to empagliflozin, linagliptin, metformin, or any of the other ingredients or excipients contained in Trijardy XR.

WARNING: LACTIC ACIDOSIS

  • Postmarketing cases of metformin-associated lactic acidosis have resulted in death, hypothermia, hypotension, and resistant bradyarrhythmias. Symptoms included malaise, myalgias, respiratory distress, somnolence, and abdominal pain. Laboratory abnormalities included elevated blood lactate levels, anion gap acidosis, increased lactate/pyruvate ratio; and metformin plasma levels generally greater than 5 mcg/mL. 
  • Risk factors include renal impairment, concomitant use of certain drugs, age greater than 65 years old, radiological studies with contrast, surgery and other procedures, hypoxic states, excessive alcohol intake, and hepatic impairment. Steps to reduce the risk of and manage metformin-associated lactic acidosis in these high risk groups are provided in the Full Prescribing Information. 
  • If lactic acidosis is suspected, discontinue TRIJARDY XR and institute general supportive measures in a hospital setting. Prompt hemodialysis is recommended.
  • Prior to starting treatment with Trijardy XR, inform your doctor:
    • Of all the prescription and nonprescription medications you take, as well as vitamins and herbal supplements.
    • Of any allergies you may have.
    • Of your medical conditions.
    • Of your pregnancy status or if you plan to become pregnant.
    • Of your breastfeeding status or if you plan to breastfeed.
  • Talk to your doctor before you undergo iodinated contrast imaging procedures. Your doctor may decide to stop Trijardy XR before or at the time of your procedure.
  • Inflammation of the pancreas can occur with the use of Trijardy XR. Immediately call your doctor if you develop stomach pain that is severe and continuous or pain from your abdomen to your back, with or without vomiting.
  • Ketoacidosis, a condition in which ketones are elevated in your blood or urine, can occur with the use of Trijardy XR. Get emergency care immediately if you have nausea, vomiting, stomach pain, breathing problems, and tiredness.
  • Dehydration can occur with the use of Trijardy XR. You may be at a higher risk if you have kidney problems, are 65 years old or older, are on a low sodium diet, or take blood pressure medications. 
  • Urinary tract infections can occur with the use of Trijardy XR. Tell your doctor immediately if you have burning when urinating, frequent urination, urge to urinate, lower stomach or pelvic pain, fever, nausea, vomiting, back pain, or blood in your urine. 
  • A bacterial infection that can lead to tissue damage under the skin around your anus and genitals can occur with the use of Trijardy XR. Get medical attention immediately if you develop a fever, weakness, fatigue, general discomfort, and pain, tenderness, swelling, and redness of the skin around your anus and genitals.
  • A vaginal yeast infection can occur with the use of Trijardy XR. Tell your doctor if you have vaginal odor, vaginal discharge that is white or yellowish, and/or itching of your vagina.
  • A penile yeast infection can occur with the use of Trijardy XR. Tell your doctor if you have penile redness, itching, or swelling, odorous penile discharge, a penile rash, or pain of the penile skin.
  • Your vitamin B12 levels can decrease with the use of Trijardy XR. 
  • Severe joint pain can occur with the use of Trijardy XR. 
  • A skin reaction can occur with the use of Trijardy XR. Immediately tell your doctor if blisters develop on your skin.
  • Heart failure can occur with the use of Trijardy XR. Immediately tell your doctor if you have breathing problems, swelling in the feet, ankles, or legs, rapid weight gain, and abnormal fatigue.

Side Effects

Trijardy XR can cause side effects such as constipation, diarrhea, headache, stomach and intestinal inflammation, stuffy or runny nose, sore throat, upper respiratory tract infection, and urinary tract infection. 

These are not all the possible side effects of Trijardy XR. Tell your doctor if you develop side effects that are troublesome or persistent.
