What is a Generic Drug?

Description

Thioridazine is a typical antipsychotic of the phenothiazine class, previously indicated for the treatment of schizophrenia and other psychotic disorders. It exerts its effects primarily through dopamine D2 receptor antagonism in the brain, leading to a reduction in psychotic symptoms such as hallucinations, delusions, and disorganized thinking. Thioridazine also possesses anticholinergic, antiadrenergic, and antihistaminic properties, contributing to both therapeutic and adverse effects. Due to serious cardiac risks, including QT prolongation and fatal arrhythmias, its use is restricted and it has been withdrawn or limited in many countries.

Thioridazine should be used only in patients with schizophrenia who have not responded adequately to other antipsychotics.

  • Typical starting dose for adults: 50–100 mg three times daily, titrated cautiously.
  • Usual maintenance dose: 200–800 mg/day, divided in 2–4 doses.
  • Maximum recommended dose: 800 mg/day
  • Use the lowest effective dose and regularly monitor for cardiac effects.
  • Elderly patients or those with hepatic/renal impairment may require dose adjustments.

Ingredients

  • Thioridazine hydrochloride, available in 10 mg, 25 mg, 50 mg, 100 mg, and 200 mg strengths

Inactive ingredients may include:

Contraindications

Thioridazine is contraindicated in:

  • Patients with congenital long QT syndrome or history of cardiac arrhythmias
  • Use with drugs known to prolong the QT interval (e.g., certain antiarrhythmics, antibiotics, SSRIs)
  • Severe CNS depression or coma
  • Known hypersensitivity to thioridazine or other phenothiazines

Cautions

Boxed Warning: Thioridazine prolongs the QT interval and can cause torsades de pointes, which may be fatal. It should only be used in treatment-resistant schizophrenia when other antipsychotics have failed. Obtain a baseline ECG and monitor potassium and magnesium levels. Use caution in patients with electrolyte imbalances, hepatic dysfunction, or cardiovascular disease. May impair judgment, especially early in treatment. Avoid alcohol and CNS depressants. Not recommended in pediatric patients or during pregnancy unless clearly indicated.

Side Effects

Common and potential side effects include:

  • Drowsiness or sedation
  • Dry mouth, blurred vision, constipation (anticholinergic effects)
  • Orthostatic hypotension
  • Extrapyramidal symptoms (tremor, dystonia, akathisia)
  • Tardive dyskinesia (risk increases with long-term use)
  • QT prolongation and risk of fatal arrhythmia
  • Rare: agranulocytosis, jaundice, neuroleptic malignant syndrome
