What is a Generic Drug?

Description

Tavneos (avacopan) is a medicine for adults with severe, active forms of ANCA-associated vasculitis, including granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA). These rare autoimmune diseases cause the immune system to attack small blood vessels, which can damage the kidneys, lungs, and other organs. Avacopan blocks a receptor involved with inflammation in these conditions. It is used along with other treatments and may help reduce the amount of steroid medicine needed.

Directions

Take Tavneos as 30 mg (three 10 mg capsules) twice daily with food. Swallow the capsules whole. Do not crush, chew, or open them. If you miss a dose, skip it and take your normal dose at the next scheduled time. Do not take two doses at once.

Before treatment starts, a healthcare provider will check your liver function and test for hepatitis B. Liver tests are usually repeated about every four weeks for the first six months. Continue taking Tavneos with your other vasculitis medicines unless your healthcare provider tells you otherwise.

Ingredients

The active ingredient in Tavneos is avacopan. Inactive ingredients may include polyethylene glycol 4000 and polyoxyl-40 hydrogenated castor oil. The capsule shell contains gelatin, red iron oxide, yellow iron oxide, and titanium dioxide, and the sealing band contains gelatin and polysorbate 80.

Cautions

  • Before using this medication, you may want to consult a healthcare provider about the following:
    • Any existing health conditions
    • Any allergies
    • A list of all medications currently being taken
  • Serious liver injury is a possible risk with Tavneos, with reported cases of vanishing bile duct syndrome. Liver blood tests are done before starting, about every 4 weeks for the first 6 months, and as needed after that.
  • Tavneos is not recommended for people with cirrhosis or severe liver impairment.
  • Older adults and people of Japanese descent may need more frequent liver monitoring.
  • Hepatitis B can flare during treatment. Everyone is screened for hepatitis B before starting, and anyone with a current or past infection should be managed by a specialist.
  • Tavneos should be avoided during an active, serious infection, including a localized one. Report fever, chills, or any other sign of infection.
  • Because Tavneos is taken along with steroids and other medications that suppress the immune system, the overall risk of infection is higher.
  • Avoid strong and moderate CYP3A4 inducers such as rifampin, carbamazepine, phenytoin, and St. John’s wort, since they can make Tavneos less effective. Strong CYP3A4 inhibitors call for a lower Tavneos dose.
  • Tavneos can increase levels of some other medications handled by the CYP3A4 enzyme, so those may need dose adjustments.
  • It is not known whether Tavneos can harm an unborn baby or pass into breast milk, so discuss pregnancy, plans to become pregnant, and breastfeeding with a healthcare provider.

Contraindications

Avoid taking or using this medication if there is a known history of a serious hypersensitivity reaction to avacopan or to any of the other ingredients in the capsule.

Common side effects of Tavneos include:

  • Nausea
  • Headache
  • Higher blood pressure
  • Diarrhea
  • Vomiting
  • Rash
  • Fatigue
  • Upper stomach pain
  • Dizziness
  • Raised creatinine on blood tests
  • Tingling or numbness in the hands or feet

Get medical help for swelling of the face, lips, tongue, or throat, trouble breathing, or hives, which can be signs of a serious allergic reaction. Contact a healthcare provider about signs of liver injury such as yellowing of the skin or eyes, dark urine, pale stools, itching, pain on the right side of the abdomen, nausea, or unusual tiredness. Fever, chills, a persistent cough, or any other sign of infection should also be reported, as Tavneos is often used together with medications that can lower the body’s ability to fight infection.
