What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.

Description

Orencia is an injectable medication prescribed to treat the following conditions:

  • Moderate to severe rheumatoid arthritis
  • Polyarticular juvenile idiopathic arthritis
  • Psoriatic arthritis

It is also used to prevent acute graft versus host disease (aGVHD) in patients 2 years old or older and are receiving hematopoietic stem cell transplantation (HSCT).

Orencia should not be used with other medications that can suppress the immune system, including biological disease-modifying antirheumatic drugs (dMARDS) and Janus kinase (JAK) inhibitors.

Orencia comes as a medication to be injected under the skin in a single-dose prefilled syringe or single-dose prefilled ClickJect™ autoinjector.

Directions

Orencia injection should be administered as your doctor has prescribed. If you have any questions or concerns, talk to your doctor or pharmacist.

If Orencia is being administered at home, your doctor will provide directions to you or your caregiver on how to properly prepare and inject Orencia, how much should be injected, and how often it should be injected.

Your prescription will come with storage instructions that should be followed. Refrigerate Orencia prefilled syringes and ClickJect autoinjectors in their original containers between 36°F and 46°F (2°C and 8°C). They should not be frozen. Keep away from light. Do not use it if the product is past its expiration date.

Ingredients

The active ingredient in Orencia is abatacept, which is a selective T cell costimulation modulator.

Inactive ingredients include the following: dibasic sodium phosphate anhydrous, monobasic sodium phosphate monohydrate, poloxamer 188, sucrose, and water for injection.

Contraindications

There are no known contraindications associated with Orencia according to its most recent Prescribing Information.

Cautions

  • Before you start receiving Orencia injections, tell your doctor:
    • About all prescription and nonprescription medications you take
    • If you have any allergies
    • About your current health conditions
    • About your past medical history
    • If you are pregnant or breastfeeding
  • Using Orencia with TNF antagonists can increase the risk of infection. If you are unsure whether you take these medications, ask your doctor.
  • Severe allergic reactions, including anaphylaxis may occur while using Orencia. If you have trouble breathing; swelling of the face, lips, tongue, or throat; a rash; or a rapid heart rate, immediately seek medical attention.
  • Serious infections may develop while being treated with Orencia. The incidence of infections may be higher in patients who have recurrent infections or a condition that increases their risk for infections.
  • Prior to beginning treatment with Orencia, patients should be screened for tuberculosis and viral hepatitis.
  • Live vaccines should not be administered to patients on Orencia or within 3 months of their last dose.
  • More frequent respiratory problems may occur in patients with COPD while using Orencia.
  • Reactivation of cytomegalovirus and Epstein-Barr Virus may occur in patients who use Orencia for prevention of aGVHD.

Side Effects

Some, but not all, side effects associated with Orencia medication include headache, upper respiratory tract infection, nasopharyngitis, and nausea. If you develop side effects that concern you or won’t go away, talk to your doctor.
