What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

Description

Ogivri is used to treat adults with certain cancers that have high levels of the HER2 protein (HER2-positive). This prescription treatment may reduce the risk of early breast cancer returning after surgery. It also effectively addresses metastatic breast cancer as well as metastatic stomach and gastroesophageal junction cancer.

Ogivri targets the HER2 protein to slow or stop tumor growth. It binds to the HER2 protein on cancer cells and blocks specific signals that allow the cancer to continue growing. This medication also supports the immune system in recognizing cancer cells, which helps the body weaken and destroy them.

Directions

You can buy Ogivri as a 150 mg single-dose vial. It only includes a powder for mixing, but no diluent. The 420 mg multiple-dose vial is available with a sterile diluent vial or as a powder for mixing only.

Your healthcare provider will first mix and then give you an Ogivri IV infusion in a hospital, infusion clinic, or another medical setting. Never administer Ogivri on your own and always follow your doctor’s advice. Ogivri is administered slowly through a drip, which usually takes 30 to 90 minutes, depending on whether it is your first dose or a follow-up dose. The following sections outline typical dosing schedules and treatment length, depending on the type of cancer being treated.

Early (Adjuvant) Breast Cancer

Your doctor may prescribe weekly or 3-weekly dosing for early breast cancer:

Once-Weekly Treatment:

  • First (loading) dose: 4 mg/kg
  • Follow-up doses: 2 mg/kg

3-Weekly Treatment:

  • Loading dose: 8 mg/kg
  • Follow-up doses: 6 mg/kg

In both cases, the treatment generally continues for up to a year.

Metastatic Breast Cancer

  • Weekly administration
  • Loading dose: 4 mg/kg
  • Follow-up doses: 2 mg/kg
  • Treatment continues until the cancer progresses

Metastatic Gastric / GEJ Cancer

  • 3-weekly administration
  • Loading dose: 8 mg/kg
  • Follow-up doses: 6 mg/kg
  • Treatment continues until the disease worsens

If you missed a dose by 1 week or less, you should receive it as soon as possible. If you missed it by more than one week, you require another full loading dose.

Contraindications

According to the FDA, there are no known contraindications of Ogivri.

Cautions

  • Before starting treatment, it’s essential that your doctor confirms your cancer as HER2-positive, as the treatment will not be effective otherwise.
  • Disclose all of your health conditions, medications, and allergies to your healthcare provider. You should also inform them of your pregnancy and breastfeeding status.
  • Always follow your healthcare provider's advice while receiving Ogivri.
  • Schedule routine check-ups with your doctor, including blood tests and physical exams.
  • Inform every healthcare provider that you are receiving Ogivri. This is especially crucial before surgeries or taking new medications.
  • Ogivri may negatively impact your heart, making regular heart function tests essential during treatment. Tell your doctor about prior anthracycline (e.g., doxorubicin) treatment, since the risk of heart problems is higher when these drugs are used before or with Ogivri.
  • Your doctor will watch you closely during infusions, especially your first one. This is to avoid severe infusion reactions, including trouble breathing and allergic reactions.
  • You must avoid pregnancy or breastfeeding while receiving Ogivri and for at least 7 months after your last dose. Ogivri can harm an unborn baby and may pass into breast milk.

Serious side effects include:

  • Weakened heart function or heart failure

  • Serious lung problems (inflammation, breathing difficulty)
  • Severe diarrhea or dehydration
