What is a Generic Drug?

Description

Marcaine (bupivacaine hydrochloride injection, USP) is a long-acting local anesthetic indicated for local or regional anesthesia, including epidural, caudal, and peripheral nerve blocks, as well as infiltration anesthesia in surgical, dental, and obstetric procedures. Bupivacaine blocks nerve impulse transmission by inhibiting sodium ion influx through voltage-gated sodium channels. Marcaine is valued for its prolonged duration of action, often exceeding that of lidocaine or mepivacaine, making it especially suitable for extended surgical procedures and postoperative pain management.

Directions

Marcaine must be administered by trained healthcare professionals experienced in regional anesthesia techniques.

  • Dosage varies based on type of block, anesthetic technique, patient age, weight, and physical status.
  • Typical doses range from 25 mg to 175 mg per administration.
  • The maximum single dose for most adults is generally 175 mg, not to exceed 400 mg in 24 hours.
  • Onset of anesthesia occurs within 15 minutes, with effects lasting 2 to 9 hours, depending on concentration and route.

Do not inject intravenously. Always aspirate before injection to avoid intravascular administration.

Ingredients

Each mL of Marcaine contains:

  • Bupivacaine hydrochloride (0.25%, 0.5%, or 0.75%)

Other components:

  • Sodium chloride for isotonicity
  • Water for injection
  • Hydrochloric acid and/or sodium hydroxide to adjust pH (range: 4.0–6.5)

Preservative-free formulations are used for spinal and certain epidural applications.

Contraindications

Marcaine is contraindicated in:

  • Patients with known hypersensitivity to bupivacaine or other amide-type local anesthetics
  • Obstetrical paracervical block due to the risk of fetal bradycardia and death
  • IV regional (Bier) block procedures due to the risk of cardiac toxicity

Cautions

Accidental intravascular or intrathecal injection can cause severe cardiovascular and CNS toxicity, including seizures, hypotension, arrhythmias, and cardiac arrest. Use with caution in patients with cardiovascular disease, hepatic impairment, elderly, or debilitated patients. Ensure resuscitative equipment and personnel are immediately available. Do not mix with other local anesthetics unless compatibility is known. Use preservative-free solutions for intrathecal administration only.
