What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.

Generics are less expensive because generic manufacturers don't have to invest large sums of money to develop a drug. When the brand-name patent expires, generic companies can manufacture a copy of the brand-name and sell it at a substantial discount.

Description

Lidex Ointment (fluocinonide 0.05%) is a high-potency topical corticosteroid indicated for the relief of inflammation and pruritus associated with corticosteroid-responsive dermatoses, including psoriasis, eczema, seborrheic dermatitis, and lichen planus. Fluocinonide exerts anti-inflammatory, antipruritic, and vasoconstrictive effects by suppressing the release of inflammatory mediators and cytokines. In Canada, it is marketed under the brand name Lyderm, with identical active ingredient concentration and clinical indications.

Directions

Apply a thin layer to the affected area once or twice daily, or as directed by a physician.

  • Gently rub in until fully absorbed.
  • Do not use with occlusive dressings unless instructed.
  • Limit treatment duration to 2 weeks for high-potency corticosteroids to minimize the risk of adverse effects.

Avoid application to the face, groin, axillae, or areas of thinning skin unless specifically directed by a healthcare provider.

Ingredients

Each gram of Lidex or Lyderm Ointment contains:

  • Fluocinonide 0.05% (high-potency synthetic corticosteroid)

Inactive ingredients (may vary slightly by brand) typically include:

  • White petrolatum and mineral oil (ointment base)

Lidex is available in cream, gel, and ointment formulations; Lyderm is marketed similarly in Canada.

Contraindications

Fluocinonide is contraindicated in patients with:

  • Hypersensitivity to fluocinonide or any component of the formulation
  • Untreated bacterial, fungal, viral, or parasitic skin infections
  • Rosacea, perioral dermatitis, or acne vulgaris

Cautions

Prolonged use may result in skin atrophy, striae, telangiectasia, or systemic corticosteroid absorption, especially when applied to large surface areas or under occlusion. Pediatric patients are more susceptible to adrenal suppression. Use caution in patients with compromised skin integrity or pre-existing skin infections. Periodically assess for signs of tachyphylaxis, and discontinue if no response is observed after 2 weeks. Use during pregnancy should be limited to small areas and short durations.

Side Effects

Potential side effects include:

  • Local burning, stinging, or itching
  • Dryness or skin thinning
  • Folliculitis or acneiform eruptions
  • Hypopigmentation
  • Striae or telangiectasia
  • Rare: perioral dermatitis, secondary infection, allergic contact dermatitis
  • Systemic effects: HPA axis suppression with prolonged use or occlusion
