What is a Generic Drug?

Description

Lamprene (clofazimine) is an oral antimycobacterial agent indicated primarily for the treatment of lepromatous leprosy and multibacillary Hansen’s disease, in combination with other antileprosy drugs such as dapsone and rifampin. Clofazimine has anti-inflammatory and bactericidal activity against Mycobacterium leprae, and is particularly effective in preventing and managing erythema nodosum leprosum (ENL), a severe immunologic complication of leprosy. It also exhibits activity against certain nontuberculous mycobacteria (NTM), such as Mycobacterium avium complex (MAC), although this use is off-label.

Clofazimine should be used under the supervision of a physician experienced in the treatment of leprosy.

  • Typical adult dose: 50 mg daily, with an additional 300 mg once monthly as part of combination therapy for multibacillary leprosy.
  • For ENL management, doses of 100–300 mg/day may be used short-term, tapered based on response.
  • Should be taken with food to improve absorption and minimize gastrointestinal irritation.

Treatment duration often extends 6–12 months or longer, depending on the regimen and clinical response.

Ingredients

Each soft gelatin capsule of Lamprene contains:

  • Clofazimine 50 mg or 100 mg

  • Gelatin, glycerol, iron oxide (colorant), and sorbitol

Lamprene capsules are typically red or reddish-brown, and lipid-soluble, accumulating in fatty tissues.

Contraindications

Lamprene is contraindicated in patients with:

  • Hypersensitivity to clofazimine or any component of the formulation
  • Gastrointestinal disorders, such as severe abdominal pain or persistent diarrhea (due to risk of enteropathy)
  • Pregnancy, unless the benefit clearly outweighs the risk

Cautions

Long-term use of clofazimine may lead to crystal deposition in intestinal tissues, potentially resulting in chronic abdominal pain, diarrhea, or intestinal obstruction. Caution is advised in patients with pre-existing GI disease. Clofazimine causes skin and body fluid discoloration (ranging from pink to dark brown), which may be distressing to patients but is reversible after discontinuation. Monitor liver and renal function during prolonged therapy. Use with caution in patients on QT-prolonging agents or with cardiac history, as clofazimine may prolong the QT interval. Not recommended in pediatric patients unless no alternatives are available.

Side Effects

Common and potential side effects include:

  • Skin discoloration (pigmentation)
  • Gastrointestinal symptoms (nausea, vomiting, abdominal pain, diarrhea)
  • Photosensitivity or ichthyosis
  • Dry skin or rash
  • Rare: enteropathy, hepatotoxicity, QT prolongation, or cardiac arrhythmias
