What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Description

Isoptin SR contains a calcium channel blocker that slows down the flow of calcium into the muscle cells of the heart and blood vessels. It can help relax blood vessels and make it easier for the heart to pump blood. Isoptin SR is approved to treat high blood pressure (hypertension) in adults.

Routes of administration

Directions

Take Isoptin SR by mouth, usually with food. The typical starting dosage is 180 mg in the morning; a healthcare provider may adjust the dose up to 240 mg taken in the morning or in the morning and evening. The maximum dosage is 480 mg per day. Swallow the tablets whole. Do not crush or chew them. Use the medication at the same time each day to help maintain its effectiveness.

Contraindications

Avoid taking or using this medication if any of the following apply:

  • Allergy to any ingredient in the medication
  • Heart attack with symptoms of heart failure (such as fluid in the lungs)
  • Severe weakness of the left side of the heart
  • Cardiogenic shock (a condition where the heart cannot pump enough blood to the body)
  • Severely low blood pressure
  • Second- or third-degree heart block
  • Sick sinus syndrome (unless you have a working pacemaker)
  • Very slow heart rate (bradycardia)
  • Atrial flutter or atrial fibrillation with an extra electrical pathway in the heart (such as Wolff-Parkinson-White or Lown-Ganong-Levine syndrome)
  • Current use of ivabradine or flibanserin

    • Whether you are pregnant or breastfeeding
  • Isoptin SR can slow your heart rate and affect how electrical signals move through the heart. In some people, this could lead to irregular heartbeats or low blood pressure.
  • If you have heart failure or a history of poor heart function, Isoptin SR may not be right for you, especially if your heart’s pumping ability is reduced. In these cases, your doctor may try other medications first or adjust your treatment.
  • This medication is processed mostly by the liver and partly through the kidneys. If you have liver or kidney problems, your body may not clear the drug as quickly, which could raise the risk of side effects. Your doctor may lower your dose and check your blood tests regularly to monitor liver and kidney function.
  • Isoptin SR may interact with many other drugs, including those used to lower cholesterol (like statins), treat anxiety (such as buspirone), or control heart rate (like digoxin or beta-blockers). These combinations can increase the risk of side effects, including very low heart rate or low blood pressure.
  • Isoptin SR may pass through the placenta or breast milk and affect your baby. It should only be used during pregnancy if the benefits outweigh the risks. If you're breastfeeding, your doctor may recommend stopping either the medication or nursing.
  • Isoptin SR is not recommended for people under 18 years old. In older adults, side effects like dizziness and constipation may happen more often.
  • If you have a neuromuscular condition such as myasthenia gravis or advanced muscular dystrophy, this drug may worsen your symptoms.
  • Alcohol may interact with the medicine and raise the risk of side effects.
  • Regular checkups, blood pressure checks, and possibly EKG tests may be needed to make sure this medication is working safely.

Side Effects

Common side effects of Isoptin SR include constipation, dizziness, nausea, and headache. Fatigue, swelling (edema), and slow heart rate (bradycardia) have also been reported in a small number of patients. In clinical trials, constipation was the most frequent side effect, affecting up to around 7% of users. While most side effects are mild, patients should let their doctor know if symptoms persist or worsen.

More serious effects such as severely low blood pressure, heart block, or worsening heart failure may occur, especially in people with existing heart problems. Rare reactions include liver enzyme changes, allergic reactions, skin rash, and vision problems. Post-marketing reports have also mentioned erectile dysfunction, muscle pain, and hypersensitivity reactions. If you experience breathing trouble, significant swelling, or an irregular heartbeat, seek medical attention.
