What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.

Generics are less expensive because generic manufacturers don't have to invest large sums of money to develop a drug. When the brand-name patent expires, generic companies can manufacture a copy of the brand-name and sell it at a substantial discount.

Description

Injectafer (Ferric Carboxymaltose) is used to treat iron-deficiency anemia in adults and children 1 year and older, non-dialysis chronic kidney disease in adults, and heart failure (NYHA class II or III) with iron deficiency in adults. This iron replacement infusion is administered intravenously, allowing the body to immediately absorb and use the iron supplement. It’s a preferred method of administration for people who don’t tolerate oral iron supplements or find them ineffective. Injectafer increases hemoglobin and energy levels and improves fatigue or shortness of breath, which are symptoms often associated with low iron.

Directions

Injectafer contains 50 mg of iron per mL. It’s available in single-dose vials in the following strengths:

  • 100 mg iron/2 mL
  • 750 mg iron/15 mL
  • 1,000 mg iron/20 mL.

Factors such as your exact cause of treatment, weight, and hemoglobin levels determine your dose:

Iron Deficiency Anemia

The most common dosing schedule of Injectafer when given to treat iron deficiency anemia is 750 mg, followed by the same amount 7 days later (a total of 1,500 mg per treatment course). This applies to most adults weighing 50 kg (110 lbs) or more. Some adult patients may also receive a single larger dose, up to 1,000, depending on their weight, instead of two separate doses. In contrast, patients weighing less than 50 kg (110 lbs) typically receive two split doses of 15 mg per kg, with at least 7 days separating both.

Heart Failure with Iron Deficiency Anemia

If you are an adult weighing under 70 kg (154 lbs):

Hemoglobin level is under 10: You may receive 1000 mg on day 1 and 500 mg in week 6.

Hemoglobin level is between 10 and 14: You may receive a single dose of 1,000 mg.

Hemoglobin level between 14 and 15: You typically will receive a single 500 mg dose of Injectafer.

If you weigh 70 kg (154 lbs) or more:

Hemoglobin level is under 10: Your doctor will typically administer 1,000 mg initially, followed by another 1,000 mg 6 weeks later.

Hemoglobin level is between 10 and 14: Your doctor may prescribe 1,000 mg on day 1 and 500 mg in week 6.

Hemoglobin level is between 14 and 15: You're likely to receive a single 500 mg dose.

Injectafer infusion is given by a healthcare professional only, either as a slow IV push or as an infusion lasting at least 15 minutes. It can be diluted in saline or given as is. During administration, a healthcare professional will monitor for skin leakage to avoid long-lasting discoloration.

Ingredients

The active ingredient in Injectafer is ferric carboxymaltose.

The inactive ingredient is water for injection. Additionally, sodium hydroxide or hydrochloric acid may be used to adjust the pH to 5.0-7.0.

Contraindications

Do not take Injectafer if you are allergic or sensitive to any of the active or inactive components of this medication.

Cautions

  • Inform your doctor of any other medical conditions, such as low phosphate levels, bone problems, kidney disease, high blood pressure, heart conditions, liver problems, current infections, or potential allergies.
  • Let them know of all the medications you’re taking, including over-the-counter drugs and herbal or vitamin supplements.
  • Always disclose your pregnancy and breastfeeding status since this changes how your doctor evaluates your and your baby’s risk.
  • Your healthcare team will monitor you during and for 30 minutes after the infusion for allergy symptoms such as rash, itching, swelling, hives, wheezing, or trouble breathing.
  • Your doctor may monitor for lowered phosphate levels in your blood. This is especially true when you are receiving repeat treatments or when you have conditions that affect vitamin absorption, nutrition, or kidney function. This is because low phosphate levels can weaken or soften bones, cause bone pain, and increase the risk of fractures.
  • Your healthcare provider will monitor your blood pressure during each visit, since Injectafer may cause temporary hypertension, which may be accompanied by flushing, dizziness or nausea.
  • Injectafer may cause some blood tests to temporarily show falsely high iron levels for 24 hours after administration.

Side Effects

Commonly reported but often manageable:

  • Nausea
  • Flushing or skin redness
  • Mild dizziness
  • Pain, swelling, or redness around the injection site.
  • Headache and vomiting (especially in children)

Serious side effects:

  • Severe allergic reactions
  • Severely high blood pressure
  • Fainting or significant dizziness during infusion
  • Low blood phosphate levels.

This list of possible Injectafer side effects is not exhaustive. Seek medical help immediately if you experience persistent or severe side effects.
