When starting Hytrin for BPH or high blood pressure, start with a 1 mg dose at bedtime, not exceeding this initial dose to reduce the risk of severe low blood pressure (hypotension). If needed, the dose can be gradually increased stepwise up to 10 mg once daily for symptom improvement, with some patients requiring up to 20 mg. Blood pressure and symptoms should be monitored closely, especially when used with other antihypertensive agents or PDE-5 inhibitors. If treatment is paused for several days, restart at the initial 1 mg dose.

Avoid taking or using this medication if you have a known hypersensitivity to terazosin or other quinazolines.
The most common side effects of Hytrin include dizziness, fatigue, drowsiness, nasal congestion, and low blood pressure when standing. Some men may also experience impotence or reduced libido. These reactions are usually mild to moderate and are more likely to occur during the first week of treatment or after dose increases. In rare cases, more serious reactions like fainting, blurry vision, or urinary incontinence may occur.
Other possible side effects include nausea, swelling in the limbs, headache, and respiratory issues like nasal or sinus congestion. Less common reactions such as chest pain, dry mouth, rash, and abnormal vision have also been reported. Rare but serious effects include allergic reactions, priapism, and intraoperative floppy iris syndrome during eye surgery.
