A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.
The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.
Generics are less expensive because generic manufacturers don't have to invest large sums of money to develop a drug. When the brand-name patent expires, generic companies can manufacture a copy of the brand-name and sell it at a substantial discount.
Fosamax Oral Solution (alendronate sodium) is a bisphosphonate indicated for the treatment and prevention of osteoporosis in postmenopausal women, treatment to increase bone mass in men with osteoporosis, and treatment of glucocorticoid-induced osteoporosis. It is also indicated for the treatment of Paget’s disease of bone in men and women. Alendronate works by inhibiting osteoclast-mediated bone resorption, thereby increasing bone mineral density and reducing the risk of fractures, particularly vertebral and hip fractures. The oral solution formulation provides an alternative for patients who have difficulty swallowing tablets.
The recommended dose for Fosamax Oral Solution is:
Osteoporosis in adults: 70 mg (once weekly)
Paget’s disease: 40 mg once daily for 6 months
Each dose should be taken on an empty stomach in the morning, at least 30 minutes before the first food, drink, or other medication of the day. The solution should be swallowed with a full glass (at least 120 mL) of plain water. Patients must remain upright (sitting or standing) for at least 30 minutes after administration to reduce the risk of esophageal irritation. Do not lie down or consume other substances during this period.
Each 75 mL unit-dose bottle of Fosamax Oral Solution contains:
Alendronate sodium equivalent to 70 mg alendronate
Inactive ingredients include:
Sodium benzoate (preservative), sodium citrate, sorbitol solution, xanthan gum, artificial raspberry flavor, and purified water
The solution is clear and colorless with a raspberry flavor.
Fosamax is contraindicated in patients with:
Abnormalities of the esophagus that delay esophageal emptying (e.g., stricture or achalasia)
Inability to stand or sit upright for at least 30 minutes
Hypocalcemia
Known hypersensitivity to alendronate or any component of the formulation
Use with caution in patients with active upper gastrointestinal disorders such as dysphagia, esophagitis, gastritis, or ulcers. Adequate calcium and vitamin D intake should be ensured during therapy. Renal function should be monitored; Fosamax is not recommended in patients with creatinine clearance <35 mL/min. Rare but serious side effects such as osteonecrosis of the jaw and atypical femur fractures have been reported, particularly with long-term use. Dental examination is advised prior to initiation in patients with poor dental hygiene or invasive dental procedures.
