What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.

Description

Epivir is an oral nucleoside analogue reverse transcriptase inhibitor that is used with other antiretroviral medications to treat HIV-1 infection. 

Lamivudine is the Epivir generic.

Directions

The instructions that come with your Epivir prescription should be carefully read and followed. Talk to your doctor or pharmacist if you have any questions or concerns. 

Epivir is available as 150 mg and 300 mg tablets. The oral solution is 10 mg per mL. 

Dosing for:

Adults:

300 mg by mouth once a day. You can either take one 300 mg tablet once a day or a 150 mg tablet twice a day, with or without food. 

Pediatric Patients 3 Months or Older: 

Taken either once or twice daily, with or without food. Dose will be determined based on body weight. 300 mg daily should not be exceeded.

Patients with Renal Impairment: 

Dose will be adjusted depending on kidney function. 

Ingredients

The active ingredient contained in Epivir is lamivudine. Inactive ingredients in the 150 mg tablet include hypromellose, magnesium stearate, microcrystalline cellulose, polyethylene glycol, polysorbate 80, sodium starch glycolate, and titanium dioxide. Inactive ingredients in the 300 mg tablet include black iron oxide, hypromellose, magnesium stearate, microcrystalline cellulose, polyethylene glycol, polysorbate 80, sodium starch glycolate, and titanium dioxide. Inactive ingredients in the oral solution include artificial strawberry and banana flavors, citric acid (anhydrous), methylparaben, propylene glycol, propylparaben, sodium citrate (dihydrate), and sucrose (200 mg).

Contraindications

Do not take Epivir if you have an allergy to lamivudine or any other ingredients contained in Epivir.

Cautions

WARNING: LACTIC ACIDOSIS AND SEVERE HEPATOMEGALY, EXACERBATIONS OF HEPATITIS B, and DIFFERENT FORMULATIONS OF EPIVIR

  • Lactic acidosis and severe hepatomegaly with steatosis, including fatal cases, have been reported with the use of nucleoside analogues. 
  • Severe acute exacerbations of hepatitis B have been reported in patients who are co-infected with hepatitis B virus (HBV) and human immunodeficiency virus (HIV-1) and have discontinued use of EPIVIR. Monitor hepatic function closely in these patients, and if appropriate, initiate anti-hepatitis B treatment. 
  • Patients with HIV-1 infection should receive only dosage forms of EPIVIR appropriate for treatment of HIV-1.
  • Before taking Epivir, tell your doctor:
    • About any medications you may take.
    • If you have any allergies.
    • About any medical conditions you may have.
    • If you are currently pregnant or plan to become pregnant.
    • If you are currently breastfeeding or plan to breastfeed.
  • Patients with a history of pancreatitis or at risk for pancreatitis should use caution with Epivir use. 
  • Immune reconstitution syndrome can occur with the use of Epivir. 
  • Redistribution/accumulation of body fat have been reported in patients treated with antiretroviral therapy.
  • In the ARROW trial, pediatric patients who were treated with Epivir oral solution and other antiretroviral oral solutions experienced lower virologic suppression rates and increased viral resistance than those treated with Epivir tablets.

Side Effects

Epivir medication side effects that may occur in adults with the use of Epivir include headache, nausea, fatigue, nasal signs and symptoms, diarrhea, and cough. 

The most common side effects in children are fever and cough.
