What is a Generic Drug?

Description

Dantrium (dantrolene sodium) is a skeletal muscle relaxant indicated for the treatment and prevention of malignant hyperthermia (MH), as well as for the management of chronic spasticity associated with conditions such as cerebral palsy, multiple sclerosis, stroke, and spinal cord injury. It acts peripherally at the level of the skeletal muscle fiber by interfering with calcium release from the sarcoplasmic reticulum, reducing muscle contractions. In the context of MH, Dantrium is a life-saving antidote, reversing the hypermetabolic crisis caused by certain anesthetics or depolarizing muscle relaxants.

Directions

Dantrium is available in oral capsule and intravenous formulation:

  • Chronic spasticity (oral): Start with 25 mg once daily, increasing gradually to a typical maintenance dose of 75–100 mg/day in divided doses, not to exceed 400 mg/day.
  • Malignant hyperthermia crisis (IV): Initial dose is 2.5 mg/kg IV, repeated as needed until symptoms resolve (maximum total dose: 10 mg/kg).
  • MH prophylaxis (oral): 4–8 mg/kg/day in divided doses for 1–2 days prior to surgery in susceptible patients.

IV formulations must be reconstituted carefully and administered through a large vein to minimize phlebitis. Transition to oral dosing post-crisis may be considered for maintenance.

Ingredients

Oral capsules (25 mg, 50 mg, or 100 mg) contain:

  • Dantrolene sodium

Inactive ingredients may include lactose, magnesium stearate, starch, and capsule components.

IV formulation (for injection):

  • Lyophilized powder in 20 mg vials (with mannitol as an excipient)
  • Reconstitute with 60 mL of sterile water for injection (preservative-free) per vial

Contraindications

Dantrium is contraindicated in:

  • Patients with active hepatic disease, including hepatitis and cirrhosis
  • Patients with known hypersensitivity to dantrolene or any of its components
  • Spasticity where loss of spasticity may lead to decreased functional capacity

Cautions

Dantrolene carries a boxed warning for potential hepatotoxicity, which may be fatal. Risk is higher with long-term use and doses >400 mg/day. Regular liver function tests (AST, ALT, bilirubin) should be performed before and during therapy. Use with caution in patients with cardiac or pulmonary disease, as it may cause respiratory or cardiovascular compromise. Dantrolene may cause photosensitivity—advise sun protection. It can also potentiate the effects of CNS depressants and muscle relaxants. In MH crises, monitor for electrolyte imbalances, acidosis, and supportive care needs.
