What is a Generic Drug?

Calan SR is a calcium channel blocker used to manage high blood pressure, which may help reduce the risk of heart attacks and strokes. Directions

Calan SR is taken by mouth, usually with food. Swallow the tablets whole—don’t crush or chew them. Healthcare providers usually start adults with a dose of 180 mg in the morning, but some people, like older adults or those who are smaller, might start with 120 mg. Your healthcare provider may increase your dose to 240 mg or 360 mg per day if needed. Only take the amount your healthcare provider tells you, and don’t stop taking it suddenly on your own.

Contraindications

Avoid taking or using this medication if any of the following apply:

  • Severe problems with the left side of your heart (severe left ventricular dysfunction)
  • Low blood pressure (systolic pressure less than 90 mm Hg) or cardiogenic shock
  • A heart rhythm problem called sick sinus syndrome, unless a pacemaker is in place
  • A serious type of heart block (second- or third-degree) without a pacemaker
  • Certain irregular heartbeats (atrial flutter or atrial fibrillation) combined with extra electrical pathways in the heart (like Wolff-Parkinson-White or Lown-Ganong-Levine syndromes)
  • Allergic reactions to Verapamil or its ingredients

  • Calan SR can affect how your heart pumps blood. It may not be safe for people with serious heart failure or certain heart rhythm problems, especially if you're already taking beta-blockers like metoprolol.
  • Some people may experience a drop in blood pressure while taking Calan SR, which can lead to dizziness or feeling faint. This is more likely when starting the medication or increasing the dose.
  • In rare cases, Calan SR may affect your liver. Your doctor may order regular blood tests to check your liver enzymes while you're on this medication, especially if you develop symptoms like fatigue, yellowing skin, or right-sided belly pain.
  • Calan SR can interact with many medications, including beta-blockers, statins, digoxin, certain antibiotics, and seizure medications. These interactions may increase side effects or change how either drug works.
  • Since your liver and kidneys help process this drug, Calan SR should be used cautiously if you have liver or kidney disease. Your doctor may adjust your dose and monitor you closely.
  • There’s limited research on using Calan SR during pregnancy. It should only be used if clearly needed. Verapamil does pass into breast milk, so breastfeeding while using this medicine may not be recommended.

Side Effects

The most common side effects of Calan SR include constipation, dizziness, nausea, headache, and low blood pressure. Some people may also experience fatigue, swelling, or a slow heart rate. These effects are generally mild and may improve with time or dosage adjustments. Rarely, elevated liver enzymes or a type of bowel blockage called paralytic ileus may occur.

More serious side effects like heart rhythm problems (such as AV block or bradycardia), congestive heart failure, or fainting are uncommon but possible, especially in those with preexisting heart issues. Rash, flushing, and skin reactions like Stevens-Johnson syndrome have also been reported in rare cases. Other infrequent effects may include vision changes, increased urination, or breast changes. Let your healthcare provider know if you notice any unusual or serious symptoms while taking Calan SR.
